P. Falciparum Infection Dynamics and Transmission to Inform Elimination (INDIE-1b)

Objectives

In the current study, the investigators will first improve access to care in all villages by implementing community-based clinical case management (CCM) (year 1). During the second year, the study will directly evaluate the effect of CCM on the human reservoir of infection as compared to three different experimental approaches aimed at reducing malaria transmission, namely: (i) Active fever screening and treatment that should detect symptomatic infections for early treatment; (ii) Mass Screening and Treatment (MSAT) that will systematically screen the whole population using point-of-care diagnostics, with infected individuals immediately treated; and (iii) Mass drug administration (MDA) that will treat the whole population with a full course of an antimalarial treatment.

The study will cover two transmission seasons and the interventions will vary by season.

In Year 1 (June-December 2019), CCM of malaria will be implemented in eight study villages as improved standard of care.

In Year 2 (2020), all villages will continue with CCM and will be divided into four study arms:

  • Arm 1 (control): CCM only.
  • Arm 2: weekly active fever screening and treatment.
  • Arm 3: monthly MSAT.
  • Arm 4: 3 monthly rounds of MDA with dihydroartemisinin-piperaquine starting during the dry season (April-June).
