Impact of the use of long-lasting impregnated mosquito nets on the control of malaria in Amazonas: Perception of the population in areas at risk and evaluation of the susceptibility profile of the insecticide vectors
Objectives
This study aims to evaluate the impact of insecticide impregnated mosquito nets on the control of malaria in Amazonas, considering aspects related to the adhesion of this tool by the resident population in areas of active transmission, and entomological aspects related to the susceptibility and resistance of the populations of anopheles in endemic areas.
According to WHO, by 2013, approximately 198 million cases were diagnosed and 584,000 deaths occurred, most of them children under 5 years of age in sub-Saharan Africa. In Brazil, 99.7% of malaria transmission was concentrated in the Amazon Region with approximately 165,000 clinical cases and 40 deaths recorded in 2013. In the state of Amazonas, which accounts for almost half of all reported cases in Brazil, 2014, 65,469 cases, with a reduction of 12% compared to 2013. Because there is no vaccine and because of the resistance of the parasite to drugs, vector control remains one of the main tools to combat malaria. In addition to the routine use of thermonebulization and indoor spraying, the use of long-lasting insecticide impregnated mosquito nets (MILD’s) has more recently been adopted. In Amazonas, the implantation of mosquito nets occurred in a massive way as of 2010, however, without previous studies to evaluate the effectiveness of the strategy, the acceptability of mosquito nets by the population, or even an evaluation of the impact of mosquito nets distributed in the control of vectors in the localities answered. Considering reports of cases of resistance of anopheline populations to pyrethroids, this study intends to investigate the presence of kdr mutation in mosquitoes from different regions of the Amazon, evaluating their susceptibility and effectiveness of the insecticide as a tool for vector control. It will be monitored through a prospective study the acceptance of the mosquito nets by the population, their durability and persistence of the insecticide in the mesh. The benefits of the combined use of intradomiciliary residual spraying and impregnated nets will also be evaluated, as strategies complete in the control of vectors. The aim of this study is to evaluate impregnated mosquito nets, their adaptation to the sociocultural and environmental characteristics of the Brazilian Amazon, improving the supply of health services, so that they are effective in controlling malaria, resulting in improvements in the health conditions of the population .
